By booking, you;


Understand that the Coaches or Organisers accept no responsibility for loss, damage or injury caused by or during attendance on any of the organised activities except where such loss, damage or injury can be shown to result directly from the negligence of the Coaches or the Organisers.


Understand that my child's photograph may be taken and used for promotional purposes only.


Will ensure my child has the correct equipment and clothing, for children above the age of 4 shin pads must be worn (football only).



PROVISION OF MEDICAL DETAILS


I authorise First Class Football & First Class Sport to obtain necessary medical treatment for the Minor if the Minor is deemed medically unfit by a medical professional and hereby, in my own behalf and on behalf of the Minor, release and hold harmless Releases in the exercises of this authority.


I represent that any medication to which the Minor is allergic or medications that the Minor is currently will be submitted in the medical details section of the booking form.


I agree that the Minor shall bring medications which the Minor is currently taking with him/her to the Event and that he/she shall consume the prescribed dosage for such medications.


EPI PEN POLICY


One of the most common medical conditions we come across is allergies. Whether it is nuts, dairy produce or hay-fever it is imperative that we know of anything that may cause your son/daughter to have an allergic reaction and any medication they are taking to deal with this. For severe allergies, children may need an Epi Pen. This is a small auto injector of Epinephrine (a.k.a adrenalin) which is used in medicine to treat anaphylactic shock. Our staff members are not currently qualified to administer Epi Pen; however they are permitted to do so if we receive a Medical Authorisation letter from the parent/guardian. The Medical Authorisation letter must contain clear instructions, be signed, dated and submitted prior to the activity.



SITE TERMS & CONDITIONS


The Site is a UK-based website owned by First Class Football and complies with the Data Protection Law of 2005 relating to the personal information you supply online. Any personal information that you provide to First Class Football whilst contracting with these entities is stored and controlled by First Class Football.

By using our services, you give us your express consent to process your personal data as described hereafter.


The data we collect:

The information collected by the aforementioned entities when you register with us at the www.firstclassfootball.com, www.firstclassfootball.co.uk,www.firstclasssport.com websites is generally used to communicate with you and improve our services where possible, and specifically, to process payments or enable third parties to carry out technical functions.


These functions may be carried out by us or appointed third parties who are bound by Data Protection covenants and must process the personal information in accordance with this Privacy Policy and the Data Protection Laws.


In addition to personal data such as name, e-mail address we also analyse data about participants in order to provide a safe shopping environment for all users. Examples of the data we may collect and analyse include the Internet protocol (IP) address used to connect your computer to the Internet, login, password, computer and connection information such as browser type and version, your operating system and platform, the full Uniform Resource Locators (URL) clickstream to, through and from our website (including date and time), cookie number and your purchase history.


In using the Site you accept that your personal data may be used for such purposes. We will not sell, distribute or disclose information about you or your personal usage of the Site without your express consent or unless required or permitted to do so by law. Please note that we do provide links to other sites, which may not be governed by this Privacy Policy and you should view the respective privacy policies of those sites for further information.


Participants have the right to access their stored data, to rectify them if required and to object to the storage of their data for legitimate purposes. We would like to keep your information up-to-date, you may change any of the basic information we keep about you at any time by e-mailing us at info@firstclassfootball.com


In addition to the information which you supply to us, information and data may be automatically collected through the use of cookies. Cookies are small text files the Site can use to recognise repeat users. You can configure your Internet browser to accept or reject certain cookies or inform you when a cookie is saved.We do not cross the information recorded in the cookies with the personal information collected at the time of your registration on the Site or with your online activity.


Some of our business partners may themselves use cookies on their own websites. We have no access or control of these cookies, therefore we cannot accept liability for any malfunctioning of your PC or its installed web browser as a result of your access to any of the websites.


We may monitor customer traffic patterns, Site usage and related Site information in order to optimise your use of the Site and we may give aggregated statistics to a reputable third-party, but these statistics will include no information personally identifying you.


We may also use the Information we gather to notify you about important functionality changes and alterations to the Site, or offers of products, services or information that might be of particular interest to you.


By submitting your Information and ticking the requisite boxes on any order form, you expressly consent to such use and transfer, to the extent permitted by applicable law.


1. BOOKINGS

1.1 You can book for any number of courses considering the participant meets the age requirements (courses are open to boys and girls in school years R - 8, for those courses; however you can only book for that player at a time.

1.2 Spaces on each course are subject to availability.

1.3 First Class Football reserves the right to stop accepting bookings at any time.1.4 In order to make a booking, you will need to provide us with debit/credit card details. If you do not have a credit/debit card, you will need to make a postal application.

1.5 The price stated is per course NOT per day. All bookings made must be for the entirety of the course.

1.6 Credit notes cannot be redeemed online. In order to use a Credit Note you will need to enclose this with your application form and send to us in the post. 1.7 All bookings are taken on a first come first served basis and full payment is required with booking. The closing date is 7 days prior to the course commencing or earlier if the course is fully subscribed.

1.8 Payments are taken in pounds sterling.


2. MEDICAL CONDITIONS

2.1 It is imperative that you inform us of any medical conditions or medication that your son/daughter requires.


3. CONFIRMATION OF BOOKING

3.1 Once your booking is complete you will receive your booking confirmation by email so please ensure you provide a valid email address when booking.

3.2 If the confirmation screen does not load or you do not receive your confirmation email, please contact us immediately to confirm that your booking has been received. If you do not receive confirmation it is unlikely that your booking has been successful so please contact us to avoid disappointment. Emails may have found their way into your "Junk" or "Spam" folders, so please check there first.


4. REFUND POLICY

4.1 More than 48 hours notice

Notification of cancellation of a booking must be received at least 48 hours prior to the booking start time. Customers who cancel their booking before the 48 hours notice period will have the following option;

  • Receive a credit for use on future bookings with no expiration
  •  Receive a refund onto the card used for payment

4.2 Less than 48 hours notice

No credits or refunds will be issued for bookings which have been cancelled within the required 48 hours notice period of the booking start time for any reason. The booking will become void and the value will be lost.

4.3 For injury or illness a medical certificate may be required and you will need to contact us prior to the course to inform us. Failure to do so will result in a Credit Note not being issued.

5. EVENT CANCELLATION

5.1 First Class Football & First Class Sport reserves the right to cancel a course at any time. In this instance you will receive a refund.


COVID-19 Policy

What to do if your child develops symptoms of COVID 19

If your child develops symptoms of COVID-19, they must not attend any sports camps or sessions and should remain at home for at least 10 days from the date when their symptoms appeared. Anyone with symptoms will be eligible for testing and this can be arranged via https://www.nhs.uk/ask-for-a-coronavirus-test or by calling 119. Refunds will be given to any member who is forced to isolate.

All other household members who remain well, must stay at home and not leave the house for 10 days. This includes anyone in your ‘Support Bubble’. The 10 day period starts from the day when the first person in the house became ill. Household members should not go to work, school or public areas and exercise should be taken within the home.

If you require help with buying groceries, other shopping or picking up medication, or walking a dog, you should ask friends or family. Alternatively, you can order your shopping online and medication by phone or online.

Household members staying at home for 10 days will greatly reduce the overall amount of infection the household could pass on to others in the community.

If you are able, move any vulnerable individuals (such as the elderly and those with underlying health conditions) out of your home, to stay with friends or family for the duration of the home isolation period.

Symptoms

The most common symptoms of coronavirus (COVID-19) are recent onset of:

 a new continuous cough

 a high temperature

 a loss of, or change in, your normal sense of taste or smell (anosmia)

For most people, coronavirus (COVID-19) will be a mild illness. If you are concerned about your child’s symptoms, or they are worsening you can seek advice from NHS 111 at https://111.nhs.uk/ or by phoning 111.

How to stop COVID-19 spreading

There are things you can do to help reduce the risk of you and anyone you live with getting ill with COVID-19.

Do

• wash your hands with soap and water often – do this for at least 20 seconds

• use hand sanitiser gel if soap and water are not available

• wash your hands as soon as you get home

• cover your mouth and nose with a tissue or your sleeve (not your hands) when you

cough or sneeze

• put used tissues in the bin immediately and wash your hands afterwards
